Incident Reporting & Learning Systems

TRAINING

Introduction

Incident Reporting & Learning System is a system of reporting Patient Safety Incidents that occur in healthcare. The main objective of the programme:

   i) Investigate or review why an incident occur.
   ii) Learn from the incident.
   iii) Take appropriate action to prevent similar incident from happening.
    iv) Share with others.

Level of Training

State Champion Training

A refresher course and update on current Patient Safety Programme in Malaysia will be done at least once every 2 year for state level patient safety champions.

Specific Programme Training

Each year, Patient Safety Unit will conduct training according to the current issue or theme selected for patient safety.


Expert Invitation and Presentation at Facility Level

Patient Safety Unit, Medical Care Quality Section, Medical Development Division, MoH, also conduct training based on request from public and private sector.


The main team of trainers are officers from the Patient Safety Unit, Medical Care Quality Section, Medical Development Division. Echo training is also conducted for State Champions from State Health Departments. The officers are usually from the Medical Programme in State Health Departments, which are responsible for patient safety and quality. A list of current officers is available on the official Patient Safety Council Portal:
