Medication Error Reporting
System (MERS)

OVERVIEW

Medication Error Reporting System (MERS) began in 2009 under the supervision of the Pharmaceutical Care Branch, Pharmacy Practice and Development Division. It aims to obtain information on occurence of Medication Error, to analyse, learn and share experience from all healthcare facilities including non-MOH in order to minimise the occurence of such error. This is in line with WHO 3rd Global Patient Safety Challenge: "Medication Without Harm" that aims to increase quality of patient safety with regard to medication management. MERS guideline was launched on 17 August 2009 and updated in 2019.
